2026-2027 Enrollment for Students New to FCUSD

To enroll your student at Folsom High School for the 2026-2027 school year, please attach the following (.pdf files only) to this FHS Enrollment Google Form*: *Please do not upload until you have all documents required or registration will be incomplete.

  • Registration form
  • Proof of Age (one of the following):
    • Certified copy of birth record or statement by the local register or county recorder certifying birth date
    • Baptism certificate 
    • Passport
    • An affidavit of the parent, guardian, or custodian of the minor, or any other means of providing the age of the child as proscribed by the governing board of the school district
  • Verification of Residency (one of the following):
    • Property tax payment receipt
    • Rental property contract, lease, or payment receipt
    • Utility service contract, statement, or payment receipt
    • Pay stub
    • Voter registration
    • Correspondence from a government agency
    • Declaration of residency executed by the parent or legal guardian of a pupil
  • Immunization Records (student name needs to be on the record)
  • Copy of IEP or 504 (if applicable)
  • Academic History
    • Most recent report card and/or 
    • Unofficial transcript and/or
    • Transfer grades (if applicable)
  • Authorization for Release of School Records (please include fax number and/or school contact email so records can be obtained
  • Course Selection Google Form for your student's grade (both student and parent need to electronically sign it):
